As a physician, you want your patients to receive the best care possible. Which is why you should ask yourself these questions before you make a referral.

  • Do you see patients with an illness or injury that has resulted in a decline in their abilities to walk or perform self-care activities?
  • Do you have repeat visits from patients due to frequent falls from increased weakness?
  • Are you concerned about sending patients home because of declining capabilities?
If you answered "yes" to any of the above questions, our Home Admission Program may be the source you need to have patients evaluated for inpatient acute care at The Rehabilitation Institute of St. Louis.

Conditions Treated
Although there are many conditions accepted by our home health program for admission to The Rehabilitation Institute of St. Louis, here are a few of the most prevalent conditions:
  • General debilitation with the presence of limited mobility and / or deficits in self-care activities (bathing, dressing, feeding)
  • Parkinson's disease and multiple sclerosis exacerbations with functional decline
  • History of stroke with increased weakness and hemiparesis due to recent illness or injury
  • Back, pelvis or lower extremity fractures not requiring surgery with subsequent loss of independent function
Admission Criteria
Before a patient qualifies for a referral to The Rehabilitation Institute of St. Louis, there are several factors that must first be addressed, including:
  • Clearly established rehabilitation diagnosis and goals
  • Needs or comorbidities that require a physician for monitoring
  • Expectations that the patient can return home after a short rehabilitation stay
  • Medicare approval for inpatient acute rehabilitation admission
  • Patients are not required to have had a three-day acute hospital stay
Referral Process
The Rehabilitation Institute of St. Louis Home Admission Program streamlines the admission process to keep paperwork at a minimum, while expediting admission for each patient's best interest.
  • A rehabilitation liaison evaluates the patient via medical record review and patient / family interview
  • Medical director reviews the evaluation and determines if the patient is appropriate for admission
  • The home admission program runs Monday through Thursday, 8:00a.m. - 4:00p.m.
What To Expect
With a focus on helping patients achieve their greatest functioning abilities, our team offers a comprehensice approach to patient care, including:
  • Three hours of physical, occupational and / or speech therapies per day
  • Assessment by board certified physiatrist daily
  • RN oversight availability 24 hours / day
  • Specially trained nurses and rehabilitation expertise
