Inpatient Referral Process
If you are a patient, family member, or clinician and would like to refer a patient to The Rehabilitation Institute of St. Louis, or request information about referring a specific patient to The Rehabilitation Institute of St. Louis, including a brochure, contact our admissions supervisor at: 314-658-3900.

Outpatient Referral Process
All patients receiving outpatient services require a physician referral prior to the start of care. The physician referral must include the patient diagnosis (relevant to rehabilitation), type of rehabilitation services required (PT, OT,ST, etc.) treatment procedures (that are provided at The Rehabilitation Institute of St. Louis), referral date and physician signature. The date of the referral must not be greater than 30 days prior to the start of care.
